Medication in School

A new OGSD Health Services form (see Health Forms below) must be completed at the beginning of each school year. Medication must be supplied by the parent or guardian in the original prescription container, and be clearly labeled with the student's name, medication name, dosage, and directions. The label must match the physician’s orders on the OGSD Health Services form. No medication, including any over-the-counter medication (e.g. Tylenol, Advil, cough syrup, eye drops, etc.), will be stored or dispensed without an OGSD Health Services form that has been completed by both the physician and parent/guardian.  All medication must be picked up by a parent/guardian at the end of the school year. 

**NOTE: Students are not allowed to transport medication to and from school. All medications must be delivered to and picked up from the School Health Office by a parent/guardian.

Immunization Requirements: 

California Shots for School Immunization Requirements

All students entering school must provide up to date on their immunizations according to California Law. Go to http://www.shotsforschool.org for detailed immunization requirements. 

Tuberculosis (TB) Risk Assessment for School Entry:

 All children enrolling into Santa Clara County schools are required to have their healthcare provider complete the Santa Clara County Public Health Department TB Risk Assessment for School Entry form​.

Oral Health Exam Requirement (TK/K)

All Kindergarten students must have their dentist complete an Oral Health Assessment Form by May 31 of his or her first school year. Dental evaluations that have happened within the 12 months before your child enters school also meet this requirement.

Head Lice 

Based on recommendations and guidelines from the American Academy of Pediatrics, the Center for Disease Control, the National Association of School Nurses, and the California Department of Public Health, Oak Grove School District has a NO live lice policy. If live lice are found on a child at school, the parent/guardian will be called and asked to pick up their child by the end of the school day. The student will remain in class until parent(s) pick up and every effort will be made to maintain the privacy of the affected student. The Parent/Guardian of a student with live lice will be asked to complete the Lice Treatment Form and return it to school, when their student returns. The procedure for returning to school, AFTER an appropriate/recommended treatment has been administered: The parent/guardian must bring their child to the office to be re-examined; Student will be admitted if NO live lice are present; If nits are present, the parent/guardian will be requested to continue daily nit removal and observation for the presence of live lice; If live lice are still present, the student will be sent home.

Illnesses

Any student with the following condition/s should not attend school. Students exhibiting these symptoms may be sent home. 

  • Fever (Over 100 degrees Fahrenheit)
  • Red eyes, with eye drainage
  • Rash
  • Vomiting/diarrhea
  • Untreated head lice
  • Child has sustained an injury that requires medical attention

 

**Students who have had a fever (100 degrees or above) cannot return to school until they have been without fever and/or diarrhea or vomiting for at least 24 hours**
